Firstly, wishing you and your families a happy Spring 2026! Though it came a bit early this year, we are grateful for the fewer snow-day related office closures this winter! We hope this message finds you healthy and prosperous! We are excited to share this newsletter with you about some information on our office and a few recent cases and clinical pearls to help you in your practice.

Our office is staffed by 2 Board Certified Prosthodontists—Dr. Avinash Bidra and Dr. Xiao Chun Li who are committed to excellence and offer only the highest quality of dental treatment. Dr. Bidra (owner) also serves as Director of the Prosthodontics Residency Program at UConn Dental School in Farmington, CT. (Go Huskies!)

Our office is located right in the center of Glastonbury and we serve patients who travel from various parts of Connecticut. Patients in our practice travel from numerous states across the country.

All on 4 (Full Arch Implant) Case of the Month!

This is an unfortunate case of fixing a failed All on 4 treatment done incorrectly in a patient. The patient had failing mandibular teeth which were all extracted, and she received the All on 4 treatment with poor quality implants and a poor quality "final bridge" in Arizona. While the initial bridge itself looked within clinically acceptable limits, all implants had generalized bone loss and began to become mobile and fail within 1 year after treatment! The patient flew from Arizona to see Dr. Bidra. After careful diagnosis and treatment planning, a revisional surgery was planned, and the bridge along with all 4 failing implants and abutments were removed. 5 new implants were placed with delayed loading to minimize risk, and the patient was eventually loaded with a full arch zirconia prosthesis with no distal cantilever. The patient is now 2 years post-treatment with stable bone levels and a stable prosthesis, and is extremely grateful!

 

Cosmetic Dentistry Case of the Month!

This patient was referred to Dr. Li by his general dentist due to severe generalized tooth wear resulting from a combination of chemical erosion and mechanical attrition. Comprehensive restorability assessment showed that the remaining dentition could be predictably maintained despite the loss of tooth structure. After systematic diagnosis and treatment planning, a full-mouth rehabilitation was completed using full-coverage restorations to re-establish function, occlusal stability, esthetics, and long-term tooth preservation. Careful attention was given to restoring the patient's vertical dimension of occlusion while achieving harmonious esthetics, speech, envelope of function, and a natural appearance. The treatment was completed within 2.5 months, and the patient was referred back to his general dentist.

 

Clinical Pearl of the Month!

Implant retrieval device — an indispensable tool in your office!

As more implants continue to be placed around the world by clinicians with varying levels of training and experience, removal of many implants becomes more of a routine procedure. In addition, implants with peri-implantitis, implants placed in other countries with no manufacturer support, and implants with fracture or failed mechanics all require removal and replacement! In prosthodontics, we term this revisional implant therapy. There are many methods for implant removal, ranging from counter-torque devices, trephine burs, surgical burs, forceps, and lasers. An implant retrieval device such as the one photographed above is a very cheap, single-use device that can be attached to a torque wrench adapter and then easily threaded into the implant, and the implant can be reverse-torqued out of bone. The remnant osteotomy or "socket" should be thoroughly curetted and left to heal for 3 to 4 months, or grafted with bone, or used for immediate implant placement. These decisions are based on clinical scenarios and evaluated on a case-by-case basis.

Nevertheless, any dentist who performs extractions in the dental office should stock a few of these single-use implant retrieval devices and a surgical-grade torque wrench, and be prepared to remove implants that are ailing or failing. In our practice, we remove over 40 implants each year for various reasons. Today, we do not save an ailing or failing old implant; we simply remove it and re-start the case to achieve a newer and more predictable outcome.
