All on 4 (Full Arch Implant) Case of the Month!
This patient was referred to Dr. Bidra by her general dentist and periodontist as she had severe periodontal disease that had been managed for many years by deep cleanings, and she was refractory to treatment. Unfortunately, all her teeth were mobile and were failing and she had severe bony destruction. Patient also had severe gummy smile that made the case more challenging! Full mouth extractions and double jaw All on 6 / All on 4 treatment was completed with immediate loading following the Teeth in a Day protocol. No bone grafting was done and bilateral pterygoid implants were placed in the maxilla for increased support.
The patient eventually received Monolithic zirconia bridges. Total time in treatment was 8 months.
Cosmetic Dentistry Case of the Month!
This patient was referred to Dr. Li by her general dentist. The patient had a history of missing multiple posterior teeth, severe generalized bruxism, resulting in generalized wear with compensatory eruption, compromised smile esthetics, altered occlusal plane, and loss of VDO. Through conservative preparations adhering to prosthodontic principles, a majority of the teeth were successfully preserved without any need for endodontic treatment. Vertical dimension of occlusion was increased and a full mouth diagnostic waxup was performed to confirm and then a combination of implant and natural teeth supported zirconia crowns were employed to restore the occlusion, function and esthetics. Total time in treatment was 8 months.
Clinical Pearl of the Month!
Make a copy abutment before cementing any implant restoration!
What is a copy abutment? A copy abutment is an abutment that you can generate chairside by filling your implant crown or bridge with any kind of a quick setting material like silicone, lab putty etc. The purpose of a copy abutment is prevent excess residual cement in the mouth during cementation of implant crowns or bridges.
Any residual cement that is left uncleaned can lead to harmful biological complications (like cement induced peri-implantitis), esthetic complications, implant failure and even medical-legal complications.
Once the copy abutment is fabricated, and you are ready to cement your crown in the mouth (after the abutment is torqued and sealed), use “just enough cement” in the crown and quickly cement your implant crown over the copy abutment, clean up all the excess cement around the copy abutment, remove and quickly insert the crown over the actual abutment in the mouth and have the patient bite down into occlusion. This will GUARANTEE that you will have very minimal (if any) residual cement in the mouth! After the cement is set, do take a peri-apical radiograph after cementation to ensure no proximal cement remains and for proper record keeping.
All the best with your next crown cementation!
